This tour covers the main sections and features of TX ClientTrack. Click on the images to view larger.
From the User Dashboard, staff can view news from the HMIS team and their administrators as well as active enrollments and caseloads.
The Client Dashboard in the Client Track section allows staff to see the client’s demographics, all enrollments, services provided, as well as client goals, if any have been assigned.
PROGRAM ENTRY WORKFLOW
The client intake form begins the enrollment process. Staff enters a client's personally identifiable information (PII) including household members and emergency contacts, demographic data, and electronic release information.
Residential programs use HMIS to maintain records for various housing facilities as well as track check-in history.
Case managers can set short-term and long-term goals for clients. Staff can set target dates and document progress toward acheiving those goals.
Shelter Track providies a visual representation of each facility. Shelter staff can see who is currently assigned to each bed and manage bed assignments.
